Infertility Testing & Diagnosis
The causes of infertility are not always clear. Research has shown that fertility problems occur for three main reasons: female factor infertility, male factor infertility, and unexplained infertility. In some cases, there is more than one reason for fertility issues. Before pregnancy can be successful, underlying health problems may need to be addressed.

Fertility Tests for Women
Successful fertility in women heavily relies on her ability to release eggs from the ovaries. When the reproductive tract is working properly, an egg is able to pass into the fallopian tubes and join with sperm where it is then fertilized. Once fertilization has occurred, the egg must continue its travels to the uterus and implant in the lining. Tests for diagnosing female infertility typically include looking at these processes to see if one or multiple are impaired.
Standard testing for infertility often includes a general physical exam, as well as a gynecological exam. Other types of fertility tests for women may include:
- Ovulation Testing: During ovulation testing, a blood test is performed to determine if you are ovulating.
- Hysterosalpingography: Hysterosalpingography is a test that evaluates the condition of the fallopian tubes and uterus. It can also be used to look for possible blockages and similar issues. During the procedure, an X-ray is taken to look at the uterus after a contrast is injected to ensure that the fluids exit the fallopian tubes normally.
- Ovarian Reserve Testing: This type of test helps determine the quantity and quality of eggs for ovulation. Ovarian reserve testing often involves hormone testing that is performed early in the menstrual cycle.
- Additional Hormone Testing: Additional hormone testing may be performed to check the levels of ovulatory hormones and the pituitary hormones responsible for reproductive functions.
- Imaging Tests: A pelvic ultrasound can be used to look for diseases of the fallopian tubes or uterus. A hysterosonography can also be used to get a closer look at the uterus which is not achievable by ultrasound alone.
In rare instances, other types of tests may be performed, such as:
- Hysteroscopy: A hysteroscopy may be performed to look for diseases of the fallopian tubes or uterus if you present with certain symptoms. During the procedure, a thin, lighted device is inserted into the uterus through the cervix to look for possible abnormalities.
- Laparoscopy: Laparoscopy is a minimally-invasive procedure used to identify endometriosis, blockages, scarring, or other irregularities within the fallopian tubes, uterus, or ovaries. During the procedure, a small incision is made under the navel and a thin viewing device is inserted to examine the fallopian tubes, uterus, and ovaries.
- Genetic Testing: Your doctor may suggest genetic testing to determine if there is a genetic defect that is contributing to your infertility.
Depending on your symptoms, how long you have suffered from fertility problems, your age, and health history, you may not need all of these tests. With the help of your doctor, you can create a treatment plan that works for you and your unique situation.
Fertility Tests for Men
Fertility tests for men generally focus on whether the testicles produce sufficient healthy sperm and that the sperm is able to effectively travel from a woman’s vagina to the egg for fertilization. Tests are used to determine if any of the processes that help lead to a healthy pregnancy are impaired.
Tests to examine fertility in men may include a general physical exam, in addition to an examination of the genitals. Other fertility tests for men may include:
- Semen Analysis: As part of your fertility testing, your doctor may ask for one or more semen samples. These samples can be obtained through masturbating or by ejaculating into a clean container during intercourse. Once a sample has been collected, it is sent to a lab where it is analyzed.
- Hormone Testing: A blood test may be used to determine the level of testosterone and other male hormones in the body.
- Genetic Testing: Your doctor may suggest genetic testing to determine if there is a genetic defect causing the fertility problems.
- Testicular Biopsy: In certain cases, a testicular biopsy is performed to identify possible abnormalities that may be causing the infertility. A biopsy can also be used to retrieve sperm for various assisted reproductive therapies, such as IVF.
- Imaging: Certain imaging studies may be performed, such as a brain MRI, scrotal or transrectal ultrasound, bone mineral density scan, or a vasography, a test of the vas deferens.
- Specialty Testing: In some instances, other tests may be performed to evaluate the quality of a patient’s sperm, such as when a sperm sample is tested for DNA abnormalities.
Performing a thorough evaluation of both individuals in a couple is critical to determine the proper course of treatment. After a detailed exam, our clinicians will usually have the information needed to effectively counsel individuals or couples about their infertility challenges, as well as review various fertility treatment options that may boost their odds of a healthy pregnancy.
Developing a Treatment Plan
When evaluating a woman for fertility problems, her menstrual cycle is tracked and tests are performed at various stages. These tests provide a number of functions, such as assessing ovulation status, hormone levels, luteal function, and the function of the fallopian tubes and uterus. During this time, the man is also evaluated via a semen analysis.
Once the evaluation has been completed, the individual or couple will meet with a clinician to review the results and develop a treatment plan that suits the diagnosis and the couple’s family planning objectives.
